Ongoing Recruitment - Health Care Coordinator
Primary Duties/ Responsibilities:
- Provide timely outpatient support services per Individual Service Plans as directed, supervised, or guided by their Program Manager or Clinical Supervisor, which may include but not limited to case management, supportive counseling, skills training, crisis intervention and service coordination, and document these services using DAP format in the electronic health record.
- Complete Core Behavioral Health Assessments and updates, documenting needs in compliance with agency standards.
- Develop and regularly update individualized treatment plans based on assessed needs and member progress.
- Meet key performance indicators and production standards for the caseload.
- Coordinate discharge planning for members which may include but not limited to coordinating safe transportation for members to necessary appointments through AHCCCS, company (CHA) vehicle, or own vehicle.
- Refer members to appropriate care levels, monitor services per DBHS/RBHA criteria, and document treatment outcomes within an Adult Recovery Team Meeting/ Child and Family Team Meeting setting.
- Assist members with scheduling, attending, and following up on physical health care services and screenings.
- Conduct home and community visits, attend team meetings, and travel locally using a personal vehicle.
- Coordinate services with Program Managers, Clinical Supervisors, and local agencies, including law enforcement and Probation Departments.
- Complete additional assessments (LOCUS, CALOCUS, ASAM) to determine care levels and monitor progress.
- Comply with Community Health Associates policies and regulatory requirements, including HIPAA.
- Interact professionally with colleagues, members, and community agency staff.
- Provide care that respects member rights, cultural differences, and promotes dignity and independence.
- Attend clinical supervision meetings for 1 hour every other week or more if recommended by the clinical supervisor.
- Perform special assignments and other duties as assigned.
